CANDIDATES ELIGIBILITY DETAILS
- Assistant Medical Officer :
- M.B.B.S Degree from recognized university + 01 year of experience in the respective field.
- Junior Clerk :
- Possess a Degree in any discipline from the recognized university.
- Certificate in Basic computer Knowledge from any Govt. institute.
- Govt. Certificate in computer typing 35 wpm in English or 30 wpm in Hindi Typing.
- Staff Nurse :
- Bachelor’s degree in Nursing or 03 year Diploma in Nursing/GNM from a recognized Institution and Registration with Nursing Council of India/State.
- Sanitary Inspector:
- HSC with Sanitary Inspector’s Course Certificate from Government recognized Institute.

- Birth certificate (Matriculation Certificate).
- Caste Certificate, if you belong to reserved category.
- Domicile Certificate

- Aadhar Card/ Any other ID Card.
- 2 self-addressed envelope duly affixed with Rs. 10/- postal stamp.
- 3 latest passport size photograph duly attested by self on back side of photograph.
- Attach other necessary documents with the application form.
GENERAL CONDITIONS AND INSTRUCTIONS
- Candidates must clearly subscribed the word, “Application for the post of _____ in Category_______” on the top of the envelope while sending the application form.
- Fill all the columns of the application yourself, In plain and Capital letters.
- No cutting or over harvesting in the application.
- Incomplete, incorrect, wrongly filled, over writing, without signature, without photograph application will be rejected.
