• General Nursing and Midwifery (GNM)-RNRM from a recognized Institute. OR
• B.Sc. (Nursing) with Certificate in Community Health for Nurses (CCHN) OR
• Post Basic B.Sc. (Nursing) course with Certificate in Community Health for Nurses (CCHN)
• Candidate must be registered as Nurse and Midwifery from Nurses & Midwives Council and have valid registration certificate.

UP NHM CHO Written Exam

The Computer based Online Test will be held for duration of 2 hours (in one sitting) consisting of two sections of total 100 marks. Section-I (80 Marks) will consist of will consist of Professional Knowledge (Discipline related) and Section-II (20 Marks) will consist of General Aptitude, Reasoning, General Awareness and Basic Computer knowledge with 100 multiple choice questions (MCQ).

Each question will carry 1 mark and there is NO NEGATIVE marking for wrong answer. No marks will be awarded for un-attempted questions. The question paper shall be set in bilingual language i.e. English and Hindi only. However, in case of any inconsistency in Hindi version, the English version of the question shall be valid & final.

UP NHM CHO Online Form 2022

• UP NHM CHO 5000 Post Recruitment 2022 Candidate Can Apply Online.
• Candidate Read the Notification Before Apply the Recruitment Application Form in UP NHM CHO 5000 Post Recruitment 2022.
• Check and College the All Document – Eligibility, ID Proof, Address Details, Basic Details.
• Ready Scan Document Related to Recruitment Form – Photo, Sign, ID Proof, Etc.
• Before Submit the Application Form Must Check the Preview and All Column Carefully.

• i) The applicant has to sign on white paper with Black ink pen.
• ii) The signature must be of the applicant only and not of any other person. If at any stage the signature is not found to be matching with the candidate’s actual signature, the applicant’s candidature may be summarily rejected.
• iii) Please scan the signature area only and not the entire page.
