Date of Birth filled by the candidate in the UP NHM CHO online application form and the same recorded in the Matriculation/ Higher Examination Certificate will be accepted by the Commission/ Department for determining the age and no subsequent request for change will be considered or granted.

UP NHM CHO Eligibility Criteria

• B.Sc. (Nursing) with Certificate in Community Health for Nurses (CCHN) OR Post Basic B.Sc. (Nursing) course with Certificate in Community Health for Nurses (CCHN) from academic year 2020 & onward from an Indian Nursing Council/State Nursing Council.

UP NHM CHO Selection Process

Before applying, candidate should ensure that they fulfill the eligibility criteria. Candidature shall be subject to verification of details/documents when the candidate reports for UP NHM CHO Document Verification Process at allocated Center, if shortlisted.

Selection shall be purely based on merit and percentage of total marks obtained in theory and practical of final year of B.Sc. (Nursing) /Post Basic B.Sc. (Nursing) Examination. During the DVP, candidate will produce all the relevant original and two sets of self-attested photocopies of the same documents, which are uploaded at the time of filling UP NHM CHO application form.
